I've more or less got everything in place now, there might be a few tweaks, but the structure is all there. You're more than welcome to see what you can fit to it. I've separated out the different sections, and also put line breaks at significant changes in the melody. There is one chord I can't work out at all, so I've listed the individual notes I played in parenthesis!
